In the fifth episode of Prime Video's "Mr. & Mrs. Smith," titled "Do You Want Kids?", John and Jane Smith are sent on a mission to Lake Como, Italy, where they are tasked with locating and transporting a high-value target, Toby Hellinger (Ron Perlman), to a safehouse. However, the mission takes an unexpected turn when the safehouse is compromised, leading the pair to relocate and await further instructions from their mysterious organization.
The episode opens with John and Jane ambushing Toby in his luxurious home. They restrain him, place a bag over his head, and drive him to the safehouse directed by Hihi. Toby, assuming they are there to kill him, is reassured by Jane that they are actually there to help him. During the drive, John admires the beauty of Lake Como and suggests buying a place there, a sentiment Jane dismisses as something he often says during their missions.
Upon arrival at the safehouse, they are ambushed by a squad of armed attackers. Jane returns fire while John tries to escape the attackers on the open road. Their path is blocked by a wedding party, and Jane fires her pistol to clear the way, leading to a crash and a subsequent foot chase with Toby.
The trio navigates through Como's side streets, battling attackers along the way. John plans to escape via water and breaks into a family home to descend to street level. Toby, complaining about health issues, is forced by John to jump from the building. They offer a local Italian family cash to take them across the lake in their boat.
They arrive at a cottage, where John reveals he has impulsively bought the house as a surprise for Jane. While she appreciates the gesture, she also expresses her disapproval of such an impulsive and irresponsible decision, especially since the house was bought sight unseen.
At the cottage, Jane informs Hihi of their situation, and the company promises to send revised transport instructions. Hihi also has a private chat with Jane, inquiring about her happiness with her current arrangement and her advancement within the company. Jane confirms her contentment and Hihi emphasizes the importance of keeping Toby safe until morning.
As Toby wets himself during their escape, Jane finds him fresh clothes and medicine for his stomach. Toby refuses to take the medicine until John promises him a cigarette. Later, during dinner, Toby throws a tantrum when he realizes John never had any cigarettes, leading to a standoff that Jane must defuse.
The following morning, Jane spots attackers abseiling down the nearby cliffs and the trio makes a hasty escape to a dock where John has a boat moored. As the attackers ready their guns, the cottage explodes – Jane had left dynamite in the stove as a trap.
They take Toby out on the lake, where he is picked up by a seaplane as planned. Back home, Jane updates Hihi on the mission's success. When John notices Jane's refusal of wine and her recent sickness, he questions if she is pregnant. Jane denies it, leading to an argument about the feasibility of raising a child in their line of work. John suggests he could take on low-risk work, but Jane expresses her desire to move up in the organization.
The episode concludes with Jane asking Hihi if she is expected to have a child, given John's wishes. Hihi responds that what Jane expects of herself is most important. However, Hihi also reveals that the company believes Jane is ready to move up a level and asks if she would like to replace her John, leaving the future of their partnership hanging in the balance.
