Carl was accidentally shot by someone while in the woods. The shooter was a large, overweight man named Otis. Otis led Rick to a farm where Hershel, a veterinarian, lived. Hershel, treating the situation as a crisis, instructed his family to set up various medical equipment to remove the bullet from Carl’s body.
Lori was still unaware of Carl’s injury. Rick worried that Lori would be unable to accept the truth if she found out about Carl’s condition. Shane looked on with concern and comforted Rick; with Shane’s reassurance, Rick gradually calmed down.
T-Dog’s wrist injury was worsening. Dale found some antibiotics for T-Dog to take. As they sat outside the house, T-Dog and Dale talked about their life plans. Rick and Shane, both former police officers, were already the leaders of the group. T-Dog, unwilling to be a "cannon fodder," urged Dale to escape with him by car. Dale stared in shock at the rambling T-Dog—T-Dog’s forehead was burning hot with a fever. Dale soon realized that T-Dog was talking nonsense only because he was sick and feverish.
Andrea was attacked by a walker in the woods. Maggie arrived on horseback, swinging a baseball bat to kill the walker. Lori then followed Andrea on horseback as they headed toward the farm.
Carl’s condition was critical and he urgently needed surgery, which required additional medical supplies. Overwhelmed by guilt for shooting Carl, Otis volunteered to leave the farm to find the necessary supplies. Shane offered to go with Otis to a nearby hospital to search for the medical equipment.
When Lori arrived at the farm and saw Carl lying in bed, she nearly had a breakdown. Hershel, the veterinarian, had already performed one surgery on Carl. Upon learning that Hershel was only a veterinarian, Lori’s face filled with displeasure—she blamed Rick for grasping at straws and trusting a non-human doctor in desperation.
By the time Shane and Otis reached the hospital, it was already dark. Numerous walkers wandered around the outside of the hospital. Shane threw a flare to attract the walkers; as a crowd of walkers moved toward the flare, Shane and Otis took the opportunity to circle around the back of the herd and sneak into the hospital. The two successfully found the medical supplies they needed inside.
The walkers lingering outside the hospital spotted Shane and Otis as they exited the building. A horde of walkers roared and chased after them. Forced to change their route by the approaching herd, Shane and Otis fled into a narrow corridor. The walkers pursued them like a tidal wave; in a panic, the two scrambled into a corner, fumbling to lock a metal gate behind them to block the walkers. Dozens of walkers pressed against the gate from the outside, reaching their hands through the bars toward Shane and Otis.
